I’m Jelina and for the past 15 years, I’ve been helping patients like you manage their chronic conditions using medication management, disease education, nutrition, and mindset techniques that are essential to every person to achieve successful, healthy lives.

 

But it wasn’t always easy.  The first few years of my health journey, I was stuck in the same place you might be stuck now:  I was warned that it was only a matter of time before I was going to have diabetes because I was overweight, have a family history of diabetes and I was feeling frustrated that I was eating healthy, exercising for hours but nothing was working.

 

Finally, I had a friend in my book club who lost 60 pounds and I asked her secret.  She was still drinking wine and not scrutinizing what she was eating!  She guided me to some resources and before I knew it, I was doing research on how food affects our body, learned mindset techniques and the food industry’s role on the current diet.  I saw my weight drop effortlessly, my mood improved, and I had more time because I was not working out as much.  I implemented multiple techniques, some were successful, and others were not.  I took the successful techniques and applied it to my patients who had Type 2 Diabetes and they had PROFOUND results where several stopped needing insulin, reversed their diabetes and lost 30-40 pounds.

A 3 month holistic health coaching program for people with Type 2 diabetes or prediabetes who are ready to transform into the happiest and healthiest version of themselves without dealing with crazy diets or tons of medications.

 

This is not a program where you are required to eat a bunch of unfamiliar foods and cook a bunch of recipes you would not normally eat, count calories, workout for hours a day or be required to take a massive amount of medications, only so that you go back to your old habits because the new ones were too difficult to sustain.

 

This is a program where we work together to adjust your current meals so you don’t feel like you’re giving up your favorite meals and help address your personal obstacles and struggles to ensure long term success.Where you are treated as a WHOLE person not just the one biological aspect of your diabetes.

Here’s what your 3 months in Type 2 Transformation will look like:

 
  • You will learn about how diabetes affects your body, how to deal with low blood sugars, why you have cravings, the recommended types of foods, portion sizes and simple exercise options.

    In phase one you will learn:

    • About Diabetes and how it affects your body

    • How to deal with low blood sugars

    • Why you have cravings

    • Which foods are nutritious and the portion sizes

    • Exercise recommendations for the best results (you won’t even need a gym membership or ANY equipment)

  • After learning what leads to success in the program, we observe your natural tendencies. No changes are required during this phase, but you are asked to be aware of how you feel, and notice your current behaviors especially in regards to food.

    In phase 2 you will learn:

    • What feelings you have when eating

    • How you eat in comparison to what is recommended

    • How do you feel in the morning or throughout the day

    • What are potential triggers and obstacles for you in this journey

  • In the first two modules you learn about your disease, proper eating, and exercise habits and then you learn about how you feel and what obstacles or triggers you face in the next module, now we are going to try to start the new habits.

    In phase 3 you will learn:

    • What your blood sugars mean

    • How to impact your blood sugars

    • Personalized plan to address triggers and obstacles

    • How to resist cravings

  • After trying on the new habits of eating, exercising, and learning how to monitor your blood sugars, it is time to make the program more tailored to your daily life.

    In phase 4 you will learn:

    • How to adapt your plan to conquer triggers and obstacles

    • How to modify your diet for eating out at a restaurant, holiday meals, social gatherings or vacations

    • Mindfulness techniques that further support your process

    • Try new foods or recipes (optional, but highly encouraged—makes the process more fun)

  • Once you learn the plan that best works for you, it is time to visit your provider and get the blood work and celebrate your results!

    In phase 5 you will learn:

    • How to continue this process after graduation from the program

    • How to celebrate but not disrupt your goals

  • One 60 minute assessments and education:  

    This first meeting allows us to assess your personal goals and discuss your current medication regimen (discuss if any changes are needed), educate you about diabetes, cravings, nutrition, and exercise.  We discuss how the appointments look moving forward.  The next week you will record your blood sugars for assessment for the following appointment and observe your current lifestyle habits and feelings.

  • Nine 45 minute 1:1 sessions:

    This allows for a deep dive into your personal struggles and questions when implementing the program.  We discuss specific adjustments you can make to help refine your blood sugars.  This really allows us to pinpoint your pain points and address them head on with solutions we collaborate on.  These sessions will give guidance, supportive accountability, feedback and answer any questions you have to help you reach your goal. 

  • Worksheets and Handouts:

    You will be given worksheets on how to handle low blood sugars, the plate method, recommended portions, the meaning of blood sugars and a blood sugar log.  These will be great references as you go through the program

  • Journal Prompts:

    You will also be given journal prompts to address the mental, psychology and spiritual aspect of the disease.  This helps us assess what may drive your current behaviors and motivate you to make lasting changes.

FAQs

Here’s the questions my top clients asked before joining Type 2 Transformation:

  • Most doctor’s offices do not have the time to educate a person properly as they are often rushed patient to patient. I will take the time to not only explain diabetes, but also answer any questions. The program is meant to be a collaborative process where we take your likes and dislikes into account to provide the most successful results for you. We will go into the neuroscience and biology of cravings, so you know how to address those issues. Also, you will be given clear guidance on what to eat. The amount of effort you dedicate into this program, will determine your success. If you are all in, you will see results much quicker.

  • I am not a huge fan of giving my patient’s meal plans. I believe that change happens when we take gradual steps. By making small adjustments on meals you currently enjoy, you are more likely to stick to the program and still take pleasure in your meals. Although I will encourage you to try new foods, I want you to still delight in your meals. I offer guidance on portion sizes and the ideal way your plate should look so that no matter where you are, you know you can make a nourishing choice for your body.

  • The initial appointment is 60 minutes, but after that they are at most 30 minutes about once a week or twice a month. In the beginning, it may take a bit more time to prepare food or pay attention to what you are doing because just like starting a new job, you are learning so it may take more time. But after a few weeks, things will become second nature and you will not require as much time. We will also discuss ways to adapt the program to your lifestyle to help you get optimal results without adding time. Also, this program is about your effort. You will get out of it as much as you put into it. The more time you spend figuring things out, the more time you will save later and you will see the results quicker.

  • You are welcome to participate in the Mindful Meds program to get a better handle on your medications, but unfortunately, there are no other programs available at this time. To ensure that you get the most of the program and due to its personalized nature, it is best to invest fully so we can take a look completely at your situation and address any questions, concerns and obstacles as they arise. I hope to have an on-line course soon which would be less expensive alternative.
